At this year's Outcomes Conference, we featured several presentations that highlighted some of the current challenges within the pharmacy benefit management industry. True to our history of innovation, our speakers also offered some unique and forward-thinking ideas about creating new approaches to those challenges, while staying true to our mission of enabling better health and value in the pharmacy benefit.

Express Scripts 2007 Drug Trend Report

Dr. Brian Seiz discussed the factors affecting trend growth that are reported in the 12th edition of the Express Scripts Drug Trend Report. Last year we saw the lowest trend rate for non-specialty medications since 1994 ― 4.7%, down 1.2% from 2006. Two key factors contributed to the 2007 trend. The first is an increase in the concerns around drug safety. Numerous products have been removed from the market over the past few years because of safety concerns and the FDA came under extreme scrutiny. The bar has since been raised and the FDA is becoming more cautious with its review of new products. The second factor is a record high generic fill rate (GFR) ― 64%, up 4% from 2006. As the GFR continues to increase, drug trend will continue to decline. Over the next five years there are $65 billion in potential savings from brand-name drugs coming off patent … we still have tremendous opportunity.

Express Scripts' vision for the future involves unlocking the power of the consumer to enable better health and value. In order to achieve this, we need to be able to understand how consumers think and act with respect to prescription drugs. Dr. Emily Cox discussed recent findings among the way consumers think about and respond to generic drugs. There are billions of dollars in savings that could be realized from increased generic drug usage. But, could consumer's perceptions around generics be a barrier?

Dr. Steve Miller discusses the drivers of rapidly accelerating specialty spend, Express Scripts’ efforts supporting congressional passage of biogenerics legislation, and the best ways to make expensive specialty drugs available to patients.

Trend management is obviously important to plan sponsors, but how do cost-cutting programs impact clinical outcomes? Dr. Ed Weisbart reports on new learnings derived from three Express Scripts clinical studies. The first focused on the clinical impact of financial incentives accompanying 2006 formulary changes in the antihyperlipidemic class. The second examined the importance of drug persistence in trend management, and the third looked at how consumer-directed healthcare plans affect consumer choice and persistence.
